If your are affiliated of any of the social
security system entities ARS, EPS, ISS, have SISBEN or
Prepaid Medicine or are with an entity adapted with the
teaching profession, air forces or others, you should
bring the following documents for your entry to the Hospital,
depending on the requirements of the entity to which you
belong.: I.D. (identity card or birth certificate for
a minor) in original and photocopy.
In case it is required, bring the payment slips of the
moderating fee, copayment or bonus.
SIf you appear in the system with many affiliations, you
should show the resignation letter.
NOTE: If you are a foreign patient, you will need your
foreign ID or passport. |
